You should check your most recent pay advice to confirm that the following information is correct:

• Your legal name
• Your current home address
  

WHAT IF MY INFORMATION NEEDS UPDATING?
If your name has changed, please fax a copy of your updated Social Security Card with the correct name to WFMO at 504-865-6727.

WHAT IS THE DEADLINE TO MAKE CHANGES TO MY NAME OR ADDRESS?
The deadline for WFMO to receive changes is Dec. 20, 2011.

COMMON W-2 QUESTIONS ANSWERED HERE:

• 2011 W-2s will be mailed to all employees per IRS guidelines on/or before Tuesday, Jan. 31, 2012.
• Requests for duplicate Form W-2 will be available after Feb. 29 in order to allow the US Postal Service sufficient time to deliver W-2s to the designated address.
•  W-2 forms cannot be emailed, faxed or overnighted and cannot be forwarded to a third party such as the employee's accountant.
• Confidential information including employee earnings and tax information will NOT be released by telephone, fax or email.
