At Tulane Community Health Center, we want every patient to feel comfortable and prepared when they come to their appointment.
For every appointment, all patients should bring the following items:
1) Photo ID
2) Insurance Card (if applicable)
3) $30 visit fee (for patients without insurance)
4) Your medications or a list of your medications with dosage and frequency in which they are taken.
New patients should bring the following items:
1) Photo ID
2) Insurance Card (if applicable)
3) $30 visit fee (for patients without insurance)
4) Proof of income (to determine fee for service, $30 is the minimum amount)
5) Medications or list of medications as described in the previous section
6) Medical Records (this helps our physicians become familiar with your medical history)
We ask all patients to arrive early for their appointments. Established patients should arrive at least 15 minutes early for their appointment. New patients should arrive at least 30 minutes early for their appointment to allow time to complete paperwork. Our goal is to have every patient in the room with their doctor at their scheduled appointment time.
We will make copies of your identification and insurance card at every visit. This is done to reduce the occurrence of identity theft.
