Each patient can choose or will be assigned a primary care provider to lead their medical home care team. The care team is responsible for coordinating all services to meet patient care needs. We can schedule a same day or next day appointment based on each patient’s needs.

Patients can request clinical advice from a provider or nurse during and after office hours by telephone. They can also request appointments, medication refills, test results and copies of health records through the on-line patient portal.
