Registration Information

Cancellation notification needs to be made at least 2 weeks prior to the class date to be eligible for a full refund.  Late cancellations are subject to a 25% cancellation fee.

Refunds must be requested in writing within 30 days of cancellation.  No refund will be issued for a "no call no show". 

The spelling of the name provided on the registration form is how the name will appear on the Certificate of Completion and the provider card.

The Refresher course is a half-day course for those who have already taken the full ATLS Provider course and received their certification.  This course must be taken within six (6) months after the expiration of your ATLS certification.  Per the ACS-COT regulations, participants exceeding the 6 month expiration period will need to complete the 2 day Provider class.  Registrants must mail, fax or e-mail a copy of their current ATLS card to be eligible for the refresher course.

Class size is limited to 24. Attendees are accepted in the order that registration information and payment are received.

The registration fee includes all manuals.

Lunch is provided each day for the Provider and Instructor courses.

You will receive an e-mail confirmation when your registration is complete.  Registration is not complete until payment has been received.

Class materials will be sent after payment is received to the address listed on the registration form.  Mailing is free of charge.  Contact our office with your credit card information if you prefer overnight shipping.
