Louisiana law requires all entering college students to have up-to-date immunizations or immunity against measles, mumps, rubella, diphtheria, and tetanus. All incoming students must comply with the immunization program. REGISTRATION WILL BE BLOCKED FOR ANYONE WHO HAS NOT SUBMITTED PROOF OF IMMUNIZATION. If you have any questions regarding the immunization program or policy, please contact the Tulane Student Health Center at (504) 865-5255. You may also download immunization forms, health insurance forms, and waiver forms from their website.
First Pay Day: Friday, August 30, 2013
All new students awarded a stipend (teaching assistants, research assistants and fellows) should present an original social security card for payroll purposes. This includes students who are not citizens of the United States. If you do not have an original social security card, you may want to go to a local office of the Social Security Administration (before arriving at Tulane) and complete Form SS5. Three to four weeks are required for issuance of a social security card.
In addition, federal law requires that Tulane University verify employment eligibility of all students receiving stipends checks, work-study checks, or any other remuneration from Tulane. As a result, all fellows, teaching assistants, and research assistants will be required to complete Section 1 of the Form I-9 and provide original documents that establish identity and employment eligibility. If you will be receiving payment from Tulane, the following forms will be required from you so that we may process your payroll appointment. Please visit the Workforce Management website for more information. You should download the forms below, complete them, and submit them to the Student Employment Office, 200 Broadway Street, Suite 120 upon your arrival at Tulane. (Do not mail the forms.)
Please note: Stipend payments are contingent on the completion of all required forms and documentation.

In addition, the university’s policy is that all students must have health insurance coverage. This requirement can be met by subscribing to the Tulane-sponsored health insurance plan (Aetna) offered through the university. The cost for 2013-2014 will be approximately $2,000 for individual coverage for 12 months. Additional coverage for family members is available at an additional cost. To learn more about the Tulane-sponsored medical insurance plan, the deadline dates to accept/decline, and proof of other insurance, please visit the University Student Health Center website.
