If you have attended a Tulane activity, you received an email with a link to the online survey to complete an evaluation and a verification form for claiming AMA PRA Category 1 Credits™. Your certificate will be sent to the email on the online form.

Need a duplicate certificate?

Please contact us with the name of the meeting, the date and your name.

Need an Enduring Material certificate?

The credit verification form is within the Enduring Material. Please send the completed form and the certificate fee, if required, to us either by fax or email.

Our policy:

For live activities, requests for original certificates of credit will be honored up to one (1) year from the date of the educational activity.

Requests for copies of previously issued certificates will be honored without time limitations.
