Protocol Form -- Protocols should be submitted electronically to iacuc@tulane.edu.  Do not send copies or a signed version.  Please note that incomplete protocols will be returned.

Appendix A - Satellite or Study Area Registration- Please submit this form with your protocol if yes is answered for Section IV.#6.  If an amendment request asks for the use of a study area or satellite housing not approved on the parent protocol you must submit this form with the amendment.

Appendix B - Hazards - Please submit this form with your protocol if yes is answered for Section V. If an amendment request asks for the use of a personnel hazard not approved on the parent protocol you must submit this form with the amendment.

Appendix C- Breeding - Please submit this appendix if your protocol includes breeding of animals. If an amendment request asks for breeding  not approved on the parent protocol you must submit this form with the amendment.

Amendment Form – Use this form for modifications to your study. Email the completed form to iacuc@tulane.edu.  If your amendment requests breeding, hazards not identified on the original protocol or the use of a study area or satellite housing not approved on the original protocol, you must submit the appropriate appendices noted above with the amendment request.

Personnel Addition Form -- Use this form to add personnel who will be working with animals on your study.
