Protocol Form -- Protocols should be submitted electronically to iacuc@tulane.edu.  Do not send copies or a signed version.  Please note that incomplete protocols will be returned.

Appendix A - Satellite or Study Area Registration - Please submit this form with your protocol if yes is answered for Section IV. #6. If an amendment is submitted that requests the addition of a satellite facility or study area, you must also submit this form.

Appendix B - Hazards- Please submit this form with your protocol if yes is answered for Section V.  If an amendment is submitted that includes hazards not identified in the parent protocol , you must also submit this form.

Appendix C- Breeding - Please submit this form with your protocol if breeding is included.  If an amendment is submitted that requests the addition of breeding, you must also submit this form.

Amendment Form – Use this form for modifications to your study. Email the completed form to iacuc@tulane.edu.  Please see instructions above for completing Appendices A, B or C depending on the request for modifications.

Personnel Addition Form -- Use this form to add personnel who will be working with animals on your study.
