Report a Problem or Request Assistance

There comes a time when one encounters in his/her job a matter that appears to be dishonest, fraudulent, unethical, theft, waste, abuse, discrimination, harassment, or a major violation of established policies. If you would like to report a problem, please contact us by calling us directly at 865-5583. Alternately, please print and fill out the form below and FAX it to us at 862-8647. All of the information received will be treated in confidence.

Please note that if you are witnessing a theft in progress, please contact the Office of Public Safety, 865-5200 (x5200 on campus), immediately.

Your contact information (name, e-mail, phone) will be kept confidential. This information is needed to ensure that we can obtain all relevant information and contact you in case further clarification is needed. However, you may choose to not provide this information.

Please provide as much information as possible to guide us in the right direction (i.e. account number, department name, responsible person's name, description of events).
